Boarding Agreement

We are very pleased that you have entrusted us with the care of your pet(s) while you are away. The following information will help ensure everything is in order for their stay.

Instructions

When your pet arrives at our hospital, we will remove and return to you any collars/leashes or other personal items. We will immediately print a cage card I. D. with your pet’s name, as well as a name tag to go on your pet. Your pet will be inspected for fleas/ticks or other external parasites. If any parasites are found, your pet will be treated before entering our boarding facility at your expense. Pets that become ill while boarding will be treated at the discretion of the veterinarian on staff at the client’s expense. We will attempt to contact you at the emergency number to discuss any illnesses that may be noted or arise during boarding.

Proof of Vaccinations

All pets must be current on vaccinations to enter the boarding facility. This ensures your pet’s protection, as well as ensuring the safety of our other guests as well. We must have written or faxed documentation by a veterinarian. The required vaccines include: DOGS: DA2PLCPV (7 in 1), Bordatella (kennel cough), and Rabies Vaccine. CATS: FVRCP (4 in 1) and Rabies Vaccine.

Personal Items

Clean bedding will be provided for all pets boarding with us. We understand the desire to leave personal bedding with your pet, but due to cleanliness issues, we prefer to use our own bedding unless special needs of a pet’s illness require other bedding to be used (orthopedic conditions, etc). Any personal items left with pets must be labeled very plainly with permanent marker. CLINTON ANIMAL HOSPITAL IS NOT RESPONSIBLE FOR DAMAGED OR LOST PERSONAL ITEMS LEFT WITH PETS. Your pet’s boarding area is cleaned thoroughly every day (more often if needed) and all bedding is changed and washed. Some bedding brought from home may not hold up to these conditions and may become damaged.

Special Food or Medications

Unless personal food is brought, your pet will be fed Science Diet dry food according to his/her age and weight. If your pet requires a special diet brought from home, please leave specific feeding instructions with the food. Any medications your pet may need will also be given by our staff. Please leave very detailed instructions with medications and the time the next dose is to be given.

Bathing

It is recommended that all dogs receive a bath before going home. Pet hair attracts odor easily and even the cleanest dogs may pick up odors. The charge for bathing is additional and will range from $11-$26 depending on the size of your pet (please ask about the price for your pet). Dogs boarding 5 or more nights will receive a complimentary bath.

We accept Cash, Personal Check with driver’s license, American Express, Visa, Mastercard, Discover, and Care Credit. $25.00 charge for NSF checks. A deposit may be required for hospitalized pets.
